How to Fill out a Reporting Form
Required to confirm the reporting and registration
① : Enter your Name.
② : Enter your mobile number. For home telephone numbers or overseas numbers, please use the arrow buttons to search and select “other.”
③ : Enter your email address.
④ : Enter your occupation based on the current job, but you may select “other” if not applicable.
⑤ : Select affiliation based on the current job, but you may select “other” if not applicable.
⑥ : Enter the date you witnessed the incident that may adversely affect aviation security. If you are unsure of the date, it may be left blank.
⑦ : Enter the place where you witnessed the incident that may hinder aviation security such as the airport, at the plane, or another location.
⑧ : Fill out the detailed information.
⑨ : You may attach photos, videos or document forms as files.
⑩ : Enter password required for confirming the reporting and registration.
* If you have any other questions regarding filling out the reporting form, please contact the person in charge of the reporting system at the Aviation Safety Center of the Korea Transportation Safety Authority.

How to create a document template declaration
① : Leave this part blank as it will be recorded by the person in charge of reporting at the time of reciept
② : Fill out your personal information including name and address.
③ : Fill out job based on your current job. Please mark “O” if not applicable. For job field, choose an applicable job one from passenger search, cargo search, aircraft security and others
④ : Write the date of the witnessed incident that may hinder aviation security. If you are not sure of the date, it may be left blank.
⑤ : Fill out details.
⑥ : Include the date of writing the report form and your signature.
